Duties and Responsibilities:
- 5.1 Coordinate the various Clinical Pharmacy specialties with medical and nursing Personnel, and with other health care professionals
- 5.2 Acts as a resource person for the medical and nursing staff to furnish information
- concerning drugs, dosages and side effects
- 5.3 Develop and coordinate all clinical pharmacy specialties. Assist the medical staff
- with selection of drug therapy and patient monitoring
- 5.4 Participate in the development/review of management protocols of medical
- emergencies such as drug over dosages, cardiopulmonary resuscitation (CPR), etc.,
- and any other duties within the scope of the overall job summary.
- 5.5 Performs medical rounds and serves as preceptor or clinical instructor of
- undergraduate pharmacy students, as appropriate
- 5.6 Repot, Detect, and analyze adverse drug reactions.
- 5.7 Conduct drug utilization review and participates in patient care audits.
- 5.8 Report, detect, and analyze medication errors.
- 5.9 Respond to TPN ordering and consultation.
- 5.10 Participate in clinical research of drug studies.
- 5.11 Assist with the development and revision of pharmacy policies and procedures
- 5.12 Assist in the identification and referral of patients, coworkers, and other
- individuals having problems that may be related to substance abuse to appropriate
- evaluation and treatment resources.
- 5. 13 Conduct in-service presentation to physicians, nurses and other health
- professionals of drug related topics.
- 5. 14 Coach nursing staff to optimize efficiency in medication administration
- 5.15 Review patient's medications to ensure appropriate, effective, and safe use
